Each patient or parent is responsible for their account and will be asked to sign a financial release form. The patient portion of the bill - usually a co-payment, unmet deductible, or percentage of total charges - is due at time of service. This helps contain the overall cost of your care.
Filing Claims:
We will file claims for you if your insurance plan has a provider contract with the physicians. Contact the Business Office, by phone or at the time of your appointment, to determine if your insurance carrier has such an arrangement.
Insurance Card:
Be sure to bring your insurance card; we must have a copy of both sides on file to process your claim. We'll be happy to make copies for you.
Preauthorization:
If you are a member of a managed care plan requiring preauthorization from your primary care physician for office visits, you must contact your primary care physician's office to obtain the authorization form or number before your scheduled appointment. Please call the office with your authorization prior to your visit.

Medicare:
We do accept Medicare.
Medicaid:
We do accept Medicaid. All patients with Soonercare Medicaid must have a referral from their Primary Care Physician to be seen at Children's Eye Care. Call your Primary Care Physician and obtain the referral before your appointment. If you come without a referral, we will have to reschedule the appointment.
We cannot see Soonercare Medicaid patients for routine vision care. We apologize for any inconvenience.
"Vision" Care vs. "Medical" Eye Care:
- Vision care is the prescription of glasses. Poor vision is assumed to be due to a need for glasses. No effort is made to rule out or search for other causes.
- Medical eye care includes evaluation and treatment of a host of eye problems, including: eye pain, amblyopia, crossed eyes, infections, injuries, cataract, retinal problems, glaucoma, and tumors. Medical eye care may involve the prescription of glasses-for instance, in the treatment of eye strain, double vision, or crossing - but the cause of the problem is thoroughly evaluated first and other treatments are considered.
Often a patient will have a "medical" eye problem and a "vision" problem.
Vision Plans
We do not have contracts with any vision plans, so we do not file insurance with any vision plans. We will be happy to provide you with a superbill so that you may file with your vision plan.
