Parent / Legal Guardian

The person who accompanies the patient to their appointment is expected to provide current insurance information and payment. If you are not the legal guardian of the minor child, you must present written permission from the guardian to accompany the patient to their appointment. Foster parents and court-appointed guardians must present legal guardianship paperwork.

“Vision" Care vs. "Medical" Eye Care:

“Vision” care is the prescription of glasses. Poor vision is assumed to be due to a need for glasses. No effort is made to rule out or search for other causes. Most medical insurance policies do not pay for “vision” exams, such as a diagnosis of only nearsightedness or astigmatism requiring a glasses prescription. Therefore, we have a reasonable self pay rate for these exams that is collected at the time of the appointment.“Medical” eye care includes evaluation and treatment of a host of eye problems, including: eye pain, red eye, amblyopia, crossed eyes, infections, injuries, cataract, retinal problems, glaucoma, tumors, and many other complaints. Medical eye care may involve the prescription of glasses-for instance, in the treatment of eye strain, double vision, or crossing - but the cause of the problem is thoroughly evaluated first and other treatments are considered. Often a patient will have a "medical" eye problem and a "vision" problem. Medical insurance policies typically pay for these types of diagnoses.

Surgery

If a surgical procedure is needed, our business office will obtain any authorization that is required for surgery. We will also contact your insurance company to verify your benefit for outpatient surgery. You will be required to pay a down payment prior to surgery based on your out of pocket expense as stated by your insurance company. Any remaining balance on your account will be billed to you after we receive payment from your insurance company. You will have three months to pay the outstanding balance. All outpatient surgeries involve the surgeon, facility and anesthesiologist. Any information that we discuss with you will be for the surgeon only. You are responsible for discussing any financial questions regarding the remaining charges with the appropriate facility personnel.If for any reason your insurance company does not pay your claim within 60 days, you are responsible for payment in full.All balances not paid within three months will be sent to an outside collection agency. We accept checks, cash and all major credit cards. All returned checks will be assessed a $20.00 fee.
